At the Tioga County Human Services Agency, every client’s service plan must be authorized by our community-based Service Planning Team. The Team meets three times a week and reviews each service plan every six months to ensure that goals are being met. Our Service Planning Team is composed of professionals from throughout the county, including: a licensed psychologist, law enforcement officers, school district representatives, a physician, and licensed social workers. Some team members, such as school district staff, serve on a rotating basis to ensure client representation from each of the Agency’s geographic Patch units.
