Intake,
service planning and family-based generic case management, as well as a
variety of programs are provided through Family Services.
|

A single, common
application form (CAF),
provides clients simplified application for all services plus immediate
assessment of service needs through the Agency’s Service Planning Unit.
|
|
Composed of two supervisors and eight service planning workers
(cross-trained casemanagers), our Service Planning Unit is responsible
for reviewing all service applications, determining eligibility and
initiating case management for all Agency clients. Established clients,
or those requiring on-going case management, are transferred to the
geographic Patch unit
nearest their home.

Cross-trained
casemanagers provide a variety of casework services, including
administrative management, early intervention, targeted case management,
targeted service management, Act 152 services, 2176 Waiver services,
family-driven services, Patch and on-call services.
|
|
| Works
to protect children from abuse and serious neglect and implements the
Child Protective Services Law. Assistance includes parental instruction
and education, protective and preventive social counseling, emergency
homemaker services, emergency shelter placement, emergency medical
services and self help groups.

Adoption &
Kinship Services: |
|
Provides permanent placement opportunities for children up to age 18.
The agency provides adoptive and kinship home profiles for families
interested in pursuing permanent care arrangements and provides matching
services between potential families and children. The agency also
provides supervision of adoptive and kinship placements and supportive
services.

Temporary
Placement Services: |
|
|
Provides supportive, temporary placement for children removed from their
own home due to abuse or neglect. The program provides intensive
services for families in an effort to ease the conditions which
necessitated removal of the child from the home. If family reunification
is not possible, a permanent alternative home for the child is found as
soon as possible.

Emergency
Placement Services: |
|
Provides up to 30 days of residential care and supervision for children
whose immediate safety, protection and well-being are threatened unless
they are removed from the home; those who present a danger to themselves
or others by remaining in the home; and those who would run away if left
in the home. Emergency services are initiated when time does not allow
for the use of planned, Temporary Placement Services.

Homemaker
Services/Parenting: |
|
Offers programs in parenting,
homemaking and life skills to individuals and families throughout the
county.
|

| A
community center which focuses on strengthening bonds by providing
recreational and educational opportunities for families, as well as
information on local resources.
|
| A
respite education and anger management program for families. These
services are available to all Tioga County residents. Our REACH team
provides intensive support, education and therapy to families during
non-traditional work hours, with a goal of preventing abuse and/or out
of home placement.

A
state-subsidized, sliding fee day program that provides child care for
income-eligible parents who work outside the home or are in job
training. Services are offered at over 40 family and group day care
homes and five day care centers throughout the county. Fees are
dependent upon income and family size. |

collaborative board of agencies and local citizens whose mission is to
improve area youths' school readiness by providing parenting education
and other support services. |
