   We fused global technology with county techniques to create our "Patch" system, a method of human services delivery developed in the United Kingdom. Fanning out across the rural region, our cross-trained casework units serve specific geographic areas of Tioga County.

 

Created in 1993, our Patch system of human services delivery divides the county into five geographic Patch regions:
  • Cowanesque Valley
  • Blossburg
  • Mansfield
  • Wellsboro
  • Millerton/Lawrenceville

Each Patch casework unit consists of one supervisor and four to five cross-trained casemanagers. Patch areas have an average population of 10,000 people. Economies and trends are monitored closely and geographic Patch boundaries sometimes shift to accommodate changes.

  Clients are assigned to the Patch nearest their home. Casemanagers generally live within their assigned Patch, reducing transportation costs for the Agency and making services more accessible for clients.

Patch casemanagers work with local schools, churches and area organizations,  building community support networks for clients and their families.
